SSA Issues Form - New Paper Version!

				Last year, we created a reporting process for NADR members to submit problems they were having with local offices. The purpose is so that we can track these issues and report trends that may represent a systemic issue to Social Security. Thanks to the cooperation of the NADR membership, we have been able to provide detailed reports to SSA at our meetings with them. 
At the NADR Networking Event in Providence, one suggestion by members was to provide a paper version that they could use at their desk to quickly jot down issues as they arise, and enter them in the online reporting form later. 
We now have the paper version available for download. Print as many copies as you wish and keep them handy. Just don't forget to login and transfer the information to the online form when you can.  
Hopefully, this will make it easier for you to report problems you or your clients encounter and will help our communication efforts with SSA!
